When the patient is not in life-threatening danger, the next step is to perform a secondary assessment. After the initial primary survey confirms there are no immediate threats to life, you do a focused head-to-toe exam and gather a quick history to uncover non-urgent problems. This includes checking vital signs, assessing mental status, and obtaining information from the patient or bystanders (SAMPLE history: Signs and symptoms, Allergies, Medications, Past medical history, Last oral intake, Events leading up to the present illness). In this scenario, the patient is alert enough to respond to verbal stimuli but is confused, so a secondary assessment is essential to identify conditions like confusion from medical issues, dehydration, infection, or neurological problems, and to guide further management.

Immediate rapid transport would be reserved for unstable or life-threatening conditions. Opening the airway and ventilating is part of the primary survey if airway issues are present or respirations are inadequate. Administering aspirin is a treatment decision for suspected cardiac chest pain, not the next step in the assessment itself.
